Five Prizes will be awarded with an approximate retail value totaling $5,000.00 USD. Odds of winning depend on the number of eligible entries received for each drawing. Restrictions apply. The Sweepstakes will begin at 12:01:00 a.m. (EDT) on April 8, 2015 and end at 11:59:59 p.m. (EDT) on May 9, 2015. All online entries must be received no later than 11:59:59 p.m. (EDT) on May 9, 2015, and all mail-in entries must be postmarked by May 9 2015, and received by deadline for the applicable Weekly Prize Drawing. 1. SPONSOR: The Valpak Make Mom's Day Giveaway (the "Sweepstakes") is sponsored by Valpak Direct Marketing Systems, Inc., 805 Executive Center Dr W, St. Petersburg, FL 33702 ("Valpak" or "Sponsor"). Sweepstakes is not sponsored, endorsed or administered by Facebook or Twitter, nor are Facebook or Twitter associated with the Sweepstakes in any way.
   
   
2. ELIGIBILITY: Subject to the additional limitations set forth below, the Sweepstakes is open only to legal residents of the 50 United States and the District of Columbia and to legal residents of Canada (excluding Quebec) who are eighteen (18) years of age or older, or the age of majority in their state/province of residence, whichever is older at time of entry. Sweepstakes is not open to employees of Sponsor, or any of their parent companies, affiliates, subsidiaries, franchisees and advertising or promotional agencies, or to the immediate family members of, or to any persons domiciled with, any such employees. For purposes of the Sweepstakes, "immediate family members" are defined as spouses, children, siblings, parents, grandparents and grandchildren.
   
   
3. HOW TO ENTER: The Sweepstakes will begin at 12:01:00 a.m. (EDT) on April 8, 2015 and end at 11:59:59 p.m. (EDT) on May 9, 2015 (the "Sweepstakes Period"). U.S. residents may enter (a) online at www.valpak.com/mom or (b) by mail and Canadian residents may enter (a) online at www.valpak.ca/mom or (b) by mail. The online method is collectively referred to as "online" method in the Official Rules. To enter online, you must have a Valpak.com/Valpak.ca account. You must first login to your account or create a free account and must next complete the Official Online Registration form by entering all of the required information and following all posted instructions. If you wish to submit more than one online entry during the Sweepstakes Period, you must also check the "opt-in" box to indicate your consent to receive email updates about special offers and savings available through Valpak. Submit your entry by clicking on the "ENTER" button. You may withdraw your consent to receive these emails about special savings and offers from Valpak at any time by emailing us at customer_service@valpak.com and entering "Unsubscribe" into the subject line or by clicking on the "Unsubscribe" link in any promotional email you receive from us. Note, however, that we may still communicate with you using electronic means in relation to your participation in the Sweepstakes. Note also that if you unsubscribe or withdraw your consent to receive these marketing emails during the Sweepstakes Period, you will not be entitled to any further daily online entries for the remainder of the Sweepstakes Period. You may, however, continue to enter up to one (1) time per day using the mail-in entry mechanism. By submitting your online entry in the Sweepstakes, you agree to be bound by Sponsor's Visitor Agreement and Privacy Policy, which are hereby incorporated by reference and are posted on www.valpak.com/www.valpak.ca. Only one (1) online entry per day regardless of the online entry page method used may be registered from the same person, except additional online entries may be obtained by following the instructions in paragraph 4, below. To enter by regular mail, hand-print on a 3" x 5" piece of paper your name, address, city, state/province, zip code/postal code, daytime phone number (where you can be reached during normal business hours), email address and birth date. All mail-in entries must be mailed to The Valpak Make Mom's Day Giveaway, PO Box 43368, Atlanta, GA 30336. Only one (1) mail-in entry per person, per day is permitted, and each mail-in entry will result in the entrant receiving an additional six (6) entries into the Sweepstakes for the Prize drawing for a total of seven (7) entries per mail-in entry. No photocopies, mechanical reproductions, altered, illegible, garbled, inaudible, incomplete, mutilated, postage-due or forged entries will be accepted. All online and mail in entries must be received no later than 11:59:59 p.m. (EDT) on the last day of the applicable entry period. All entries become the property of Sponsor and none will be returned. In the event of a dispute in connection with the identity of the individual who submitted an online entry, the entry will be deemed to have been submitted by the authorized account holder of the email account identified on the online entry form, provided that such person satisfies all other Sweepstakes eligibility requirements. The authorized account holder is deemed to be the natural person who is assigned to an email address by an Internet access provider, service provider, or other online organization that is responsible for assigning email addresses for the domain associated with the submitted email address. Except for additional entries permitted under these Official Rules, multiple daily entries submitted by the same person will be disqualified, regardless of entry method. Any questions regarding the number of entries submitted or the identity of the individual who submitted an online entry shall be determined by Sponsor in its sole discretion whose decisions shall be final and binding, and Sponsor reserves the right to disqualify any entries by persons who submit false or misleading entry information or who Sponsor determines to be tampering with or abusing any aspect of the Sweepstakes. Subject to Section 4, there is a limit of one (1) entry per person per day during the Sweepstakes Period regardless of method of entry.
   
   
4. ADDITIONAL ENTRIES FOR THE PRIZE DRAWINGS: Individuals entering online may receive additional entries by referring a friend to the Sweepstakes (the "Refer A Friend") or by sharing with your friends on Facebook or Twitter (the "Share With Your Friends"). Referring A Friend and Share With Your Friends is not required to enter the Sweepstakes. To receive additional entries follow the instructions below. Refer A Friend: After completing the Official Online Registration form, you can provide up to five (5) unique, valid email addresses of persons with whom you have an existing personal or family relationship (as defined below) and who are at least eighteen (18) years old, or the age of majority in their state/province of residence, whichever is older. You must also obtain consent from each such person before providing his/her email address and by submitting his/her email address, you certify that you have done so and further certify that you have an existing personal or family relationship with them. Sponsor will send an email to each friend on your behalf advising him/her about the Sweepstakes. You will receive one (1) additional Sweepstakes entry for every unique, separate valid email address that you provide, up to a maximum of five (5) additional entries per day. False or fictitious and invalid email addresses will not be eligible for additional entries and providing false or fictitious and invalid email addresses may be grounds for disqualification from the Sweepstakes. For the purposes of Refer A Friend, "personal relationship" means you have had direct, voluntary, two-way communications with the Friend you have referred and it would be reasonable to conclude that you have a personal relationship taking into consideration any relevant factors such as the sharing of interests, experiences, opinions and information, the frequency of communication, the length of time since you last communicated or whether you have ever met in person. A "family relationship" means you are related to the referred Friend through a marriage, common-law partnership or any legal parent-child relationship and have had direct, voluntary, two-way communications. Share With Your Friends: After clicking the "Tell Friends" or "No Thanks" button on the Refer A Friend page, you can receive one (1) additional entry into the Sweepstakes per day either by clicking on the Tweet button or by clicking on the Facebook button and following the instructions on the Share screen. By clicking on the "Tweet" button, you will be sharing information about the Sweepstakes on your Twitter account. By clicking on the Facebook "Share" button, you will be sharing information about the Sweepstakes on your Facebook account. Sponsor reserves the right in its sole discretion to determine whether or not additional entries shall be awarded. Sweepstakes is not sponsored, endorsed or administered by Facebook, Inc. or Twitter, Inc., nor is Facebook, Inc. or Twitter, Inc. associated with the Sweepstakes in any way. You understand that you are providing information to the Sponsor and not to Facebook, Inc. or Twitter, Inc. Any questions, comments or complaints regarding the Sweepstakes must be directed to the Sponsor or its administrator, and not to Facebook, Inc. or Twitter, Inc.
   
   
5. PRIZE AND ODDS: The award of a prize is subject to verification of eligibility and compliance with these Official Rules. WEEKLY PRIZES: Five (5) total Weekly Prize Winners will each receive a check for $1,000 USD. Sweepstakes Entities (as defined below) reserve the right to substitute a prize of equal or greater value for any reason whatsoever. Prize Winners are solely responsible for payment of all federal, state/provincial and local taxes on the prize value awarded and for all costs and expenses not specified herein, related to any merchandise prize, including but not limited to delivery, installation, service activation fees, and other expenses incurred by accepting the merchandise prize are the sole responsibility of the prize winners.
   
   Prizes are awarded are provided "as is" with no warranty, condition or guarantee either express or implied by Sponsor. The Sponsor has not made, and will not be responsible or liable for, any warranty, condition, representation, or guarantee, express or implied, in fact or in law, relative to any prize, including but not limited to its quality, mechanical condition or fitness for a particular or general purpose. Odds of winning depend on the number of eligible entries received per entry period.
   
   The participants selected as a Weekly Prize Provisional Winner (as defined in Section 6) may be required to provide his/her valid Social Security Number to Sponsor for tax purposes and/or complete an IRS W-9 form (U.S. residents only) or a W8 form (Canadian residents only) in order to claim the prize. As a condition of receiving a prize, the Weekly Prize Provisional Winner may be required to complete, sign and have notarized an affidavit of eligibility, a liability release and, except where prohibited by law, a publicity release, which must be returned via mail or hand delivery within ten (10) business days of date of sender's notification postmark. Failure of the Weekly Prize Provisional Winner to timely sign, notarize and return any required affidavit and release or the return of any prize, affidavit/release or prize notification as undeliverable, or failure of the Weekly Prize Provisional Winner to claim a prize or to comply with any term or condition of these Official Rules, may result in disqualification, the forfeiture of his or her interest in the prize, and the award of the prize to a substitute Weekly Prize Provisional Winner to be randomly selected from all remaining eligible entries.
   
   Total ARV of the prizes awarded in the Sweepstakes: $5,000.00 USD.
   
   
6. SELECTION OF WINNERS/RANDOM WEEKLY DRAWINGS; ODDS: One (1) weekly prize winner ("Weekly Prize Winner") will be selected in a random drawing from among all eligible entries received during each Weekly Entry Period, as defined below, for a total of five (5) Weekly Prize Winners. Odds of winning a prize in each weekly drawing will depend on the number of eligible entries residents received by the deadline for the applicable weekly drawing.
   
   A random drawing will be conducted during regular business hours on or about April 15, 2015 for Prize drawing 1; on or about April 22, 2015 for Prize drawing 2; on or about April 29, 2015 for Prize drawing 3; on or about May 6, 2015 for Prize drawing 4; on or about May 13, 2015 for Prize drawing 5. All eligible entries not selected in a particular drawing will remain eligible for the subsequent Weekly Prize draws.
   
   Limit one (1) Prize per person.

   3	All eligible online and mail-in entries received between 12:01:00 a.m. (EDT) on April 8, 2015 and 11:59:59 p.m. (EDT) on April 28, 2015.
   4	All eligible online and mail-in entries received between 12:01:00 a.m. (EDT) on April 8, 2015 and 11:59:59 p.m. (EDT) on May 5, 2015.
   5	All eligible online entries received by 11:59:59 p.m. (EDT) on May 9, 2015 and all mail-in entries post marked by May 9, 2015 and received by May 12, 2015.

   
   
   
7. WINNER NOTIFICATION AND ACCEPTANCE: The Sponsor's fulfillment center will attempt to contact the each selected Weekly Prize Provisional Winner by telephone as outlined in section 6 above, using the telephone number provided on the selected Weekly Prize Provisional Winner's entry form and if an email is provided, Sponsor will email the Weekly Prize Provisional Winner. If Sponsor's fulfillment center is (a) unable to reach a Weekly Prize Provisional Winner in person by telephone (Sponsor's fulfillment center will attempt to reach Weekly Prize Provisional Winner up to three (3) times during a twenty-four hour period and no messages will be left) or (b) if a Weekly Prize Winner does not respond to the emailed notification according to the directions in the emailed notification within twenty-four hours of the time the email was sent by Sponsor's fulfillment center, then that Weekly Prize Provisional Winner will be disqualified and a substitute Weekly Prize Provisional Winner will be randomly selected from among the remaining eligible entries. The first eligible Weekly Prize Provisional Winner that the Sponsor successfully contacts personally by telephone or email and who claim the Weekly Prize in accordance with these Official Rules will be declared that week's Weekly Prize Winner.
   
   A Weekly Prize Winner may waive his or her right to receive a prize by returning the prize. The return of any prize, affidavit/release or prize notification as undeliverable, or a Weekly Prize Winner's failure to otherwise comply with these Official Rules will result in disqualification, the forfeiture of all interest in the prize, and selection of a substitute Weekly Prize Winner from among all remaining eligible entries received. Unless prohibited by law, acceptance of a prize constitutes permission for the Sponsor and its advertising and promotional agencies to use a Weekly Prize Winner's name, address (city and state/province), and photo, image and/or likeness for any commercial, publicity or promotional purposes without further notice, compensation, review or approval, worldwide and in perpetuity, in any and all forms of media, now known and hereafter devised, including without limitation the Internet.
   
   A PARTICIPANT, INCLUDING A PROVISIONAL WINNER, IS NOT A WINNER OF A PRIZE UNLESS AND UNTIL SPONSOR HAS COMPLETED ITS VERIFICATION OF PARTICIPANT'S ELIGIBILITY AND COMPLIANCE WITH THESE OFFICIAL RULES.
   
   If a winner is a resident of Canada, in order to win and receive the Weekly Prize he/she must also first correctly answer, unaided and without mechanical or electronic assistance, a time-limited mathematical skill-testing question administered by telephone. Failure of a Canadian Weekly Prize Provisional Winner to correctly answer the skill-testing question in the time allotted will result in disqualification, the forfeiture of his or her interest in the Weekly Prize, and the random selection of a substitute Weekly Prize Provisional Winner from among all remaining eligible entries received.
   
   Prizes will be delivered by express mail. Allow five weeks after completion of the Sweepstakes for delivery.
   
   
8. PARTICIPATION: By participating, you agree to be bound by these Official Rules and the decisions of Sponsor and any Sweepstakes judges or administrators appointed by Sponsor, which shall be final and binding in all respects. Any person attempting to defraud or in any way tamper with the Sweepstakes will be ineligible for the prize and may be prosecuted to the full extent of the law.
   
   
9. RELEASE: By participating in the Sweepstakes, each participant agrees to fully release, forever discharge and hold harmless Sponsor, Facebook and Twitter, and their respective parent companies, subsidiaries, affiliates, and franchisees, and the officers, directors, employees, agents, representatives and successors and assigns of all of the foregoing companies (collectively, the "Sweepstakes Entities") from and against any claims, costs, liabilities, losses, injuries and damages arising out of the Sweepstakes, including, but not limited to, any claims for personal injury, death, or damage to or loss of property, loss of enjoyment, or any other harm whatsoever arising out of: (1) participation in the Sweepstakes; (2) the quality, receipt, possession, use, or misuse of any prize; or (3) any travel or activity that is related to the Sweepstakes or any prize.
   
   
10. DISCLAIMERS OF LIABILITY: By entering, participants acknowledge and agree that the Sweepstakes Entities will not be liable for losses or injuries of any kind resulting from acceptance/possession/use/misuse of and/or inability to use a prize(s), the theft of prizes or mail, travel to and from or participation in any prize-related activity, participation in the Sweepstakes (including as a result of participation in the Refer A Friend or Share with Your Friends programs), printing, mailing or distribution errors, or any malfunctions of the telephone network, computer Internet system, computer dating mechanism, computer equipment, software, or any combination thereof, or for any entries that are late (including delayed data transmissions), tampered with, garbled, incomplete, misdirected, lost, postage-due, forged, illegible, inaudible, mutilated, mechanically duplicated, mass machine printed, or otherwise not in compliance with these Official Rules. Sweepstakes Entities are not responsible or liable for any injury or damage to any participant's or third person's computer or mobile device and/or their contents, related to or resulting from the Sweepstakes. Sponsor reserves the right to terminate, suspend or modify the Sweepstakes if, in its sole discretion, it determines that for any reason the Sweepstakes is not capable of running as originally planned, or fraud, technical failures, printing, mailing or distribution errors, or any other unanticipated causes or occurrences have compromised the fairness or integrity of the Sweepstakes. In the event the Sweepstakes is terminated prior to the stated end date, Sponsor reserves the right to award all prizes in a random drawing from among all eligible entries received as of the date of termination. BY ENTERING, PARTICIPANTS FURTHER ACKNOWLEDGE AND AGREE THAT EVERYTHING REGARDING THIS SWEEPSTAKES, INCLUDING THE WEBSITES, APPS, ENTRY PAGES AND THE PRIZE(S), ARE PROVIDED "AS IS" AND THAT TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE SWEEPSTAKES ENTITIES DISCLAIM ALL WARRANTIES AND CONDITIONS REGARDING THE PRIZES, WHETHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, THE IMPLIED WARRANTIES AND CONDITIONS OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR OR GENERAL USE OR PURPOSE.
    
    Sponsor will not be responsible for typographical, printing or other inadvertent errors in these Official Rules or in other materials or announcements relating to the Sweepstakes.

SWEEPSTAKES MAIL OPT-OUT REQUEST/WINNERS LIST: To request the name and hometowns of the Winners (available after June 10, 2015) or a copy of these Official Rules, send a self-addressed, stamped business-size envelope to The Valpak Make Mom's Day Giveaway, P.O. Indicate whether you are requesting the name of the Winners, or a copy of these Official Rules, or any combination of the foregoing. Residents of Vermont may omit return postage for rules.
