Services offered by Society's Assets. Living alone with limited family or community support, Diagnosis of a serious medical condition (heart failure, heart attack, respiratory illness, diabetes, MS, Arthritis, Parkinsons, etc.), Having a history of falls, Requiring assistance with Activities of Daily Living (ADLs), Experiencing occasional or routine incontinence, Having a history of mental illness, Requiring assistance with walking, transferring from bed to chair, Having a surgical wound or a pressure sore, Having a cognitive impairment.
Works with families and individuals of any age with disabilities ADA Education and Assessment, Assistive Technology, Benefits Counseling, Bill Paying Assistance, Disability Awareness Programs, Housing and Home Modifications, Independent Living Skills Training, Individual and Systems Advocacy, Information and Referral.
